Create Web Forms for Zoho Desk Ticket Replies

This article shows how to collect ticket reply details from customers using a Zoho Forms web form and route those responses into Zoho Desk as tickets or linked updates, using forms, shareable URLs/QR codes, a webhook to Desk, and Desk templates to distribute the form link. Why this matters

If you want structured customer replies instead of free-text email responses, a web form captures consistent fields such as problem description and priority and makes responses easier to process. [1]

A form link or QR code lets customers reply from any device and keeps data entry fast and accurate. [1]

Routing form submissions into Zoho Desk as new tickets or updates automates intake so agents see the information inside Desk without manual copy-paste. [2][4]

Step-by-step

  1. Step 1. Create the form in Zoho Forms: open Zoho Forms, give the form a name, use drag-and-drop to add fields you need (for example name, email, problem description, priority), then save the form. [1]
  2. Step 2. Get the public share options for the form: in the Zoho Forms Sharing/Delen tab, copy the generated URL and download the QR code (PNG) if you want printed posters or quick access from mobile devices. [1]
  3. Step 3. Decide how customers will receive the form link: add the copied form URL into email templates or ticket reply templates so agents can paste the link into replies, or include the QR code on printed materials for on-site customers. [6][1]
  4. Step 4. Prepare Zoho Desk to accept inbound data: confirm you can create tickets via the Desk tickets API endpoint at https://desk.zoho.com/api/v1/tickets and that you will use an authenticated Connection for authorization in a webhook. [2]
  5. Step 5. Create a workflow that triggers a webhook to Desk: in Zoho Desk go to Setup > Automation > Workflow Rules, click Create Workflow, choose the module and trigger conditions you need, then add an Action and select Webhook to create your own webhook. [2]
  6. Step 6. Configure the webhook to create or update tickets: set Method to POST, set URL to https://desk.zoho.com/api/v1/tickets, choose Authorization Type as Connection and select or create a Zoho Desk connection, then set Body Type to Raw, Format to JSON and map form fields into the JSON body such as departmentId, contactId, subject, description, and status. [2]
  7. Step 7. Use placeholders where you need dynamic values: when building the webhook body inside the workflow builder, insert placeholders from the triggering module so submitted values replace them at runtime. [2]
  8. Step 8. Locate departmentId or contactId values if you need to map to Desk objects: follow the Desk guidance for finding department IDs and contact IDs to use in the webhook JSON so tickets land in the correct place or link to the correct contact. [2][4]
  9. Step 9. Test end-to-end: submit the Zoho Form yourself, confirm the webhook fires and the JSON fields create a ticket or update in Zoho Desk, and refine field mapping or workflow criteria until results are correct. [2]
  10. Step 10. Publish and educate agents: add the form URL to the relevant email or ticket templates so agents can include the link in replies, and document when agents should request a form-based reply. [6][1]

Common pitfalls

  • Using the wrong authorization method in the webhook prevents Desk from accepting the POST; the webhook must use a Desk Connection for authorization. [2]
  • If you map to a contactId that doesn't exist in Desk, the ticket creation call will not link correctly; verify contact IDs beforehand. [2][4]
  • Relying only on printed QR codes without a visible URL can confuse users who prefer clicking links in email; provide both. [1]

What to check

  • Confirm the webhook uses Method: POST, URL: https://desk.zoho.com/api/v1/tickets, Authorization Type: Connection, and Body as raw JSON before enabling the workflow. [2]
  • Verify the form share URL and QR code work on desktop and mobile, and that submitted fields match the JSON placeholders used by the webhook. [1][2]
  • Make sure tickets created by the webhook appear under the intended department and, when required, link to the correct contact in Zoho Desk. [2][4]
